Starting hormone replacement: how do I use the products and what can I expect?
A practical overview of the different hormone products and how to use them
You have started hormone replacement therapy or are considering it. Below you will find an overview of the most commonly prescribed products and how to use them.
Estrogen
Through the skin
- Oestrogel®: apply the prescribed dose to the inside of your arms or thigh. Let the product air dry.
- Lenzetto®: apply the spray to the inside of your arms or thigh. Let the product air dry.
- Estrogen patch (Dermestril®, Systen®, Feminova®, Sandoz®): apply the patch to dry, clean skin 1 or 2 times per week (depending on the type of patch prescribed).
Oral tablets
Progynova®, Zumenon®, Estrofem®
These are less commonly used because they have a less favorable risk profile.
Progesterone
Utrogestan®, Progebel®
- Oral (swallowed): usually in the evening.
- Vaginal: also possible, but only after consultation.
The schedule:
- Cyclic: taken in the second half of the cycle on consecutive days. Day 1 = first day of menstruation/bleeding.
- Continuous: every day.
Testosterone
Androgel® through the skin: off-label use.
Starting dose: 5 mg testosterone per week (1 mg androgel does not equal 1 mg testosterone).
This gel is designed for men, so it is dosed far too high for women. During the consultation, we will discuss in detail how you can use this.
Vulvovaginal products
Products for internal use (in the vagina)
- Vagifem®: every day for the first 2 weeks, then 2 times per week. Insert with the applicator.
- Rewellfem®: every day for the first 2 weeks, then 2 times per week. Insert with the applicator.
- Aacifemine®: every day for the first 2 weeks, then 2 times per week.
- Intrarosa®: insert the suppository into the vagina with your finger.
Creams/gel for internal and external use
Aacifemine® and Blissel®
What can I expect?
Some women feel better quickly, others need more time and adjustments.
At the start, you may experience tender breasts, mild headaches or feel more emotional. You may also experience bleeding. This is usually temporary and resolves on its own. Bleeding that persists (3 to 6 months) requires a follow-up with your gynecologist (often with an ultrasound).
Take your products as prescribed and prepare for gradual improvement. Hormone replacement therapy works best alongside a healthy lifestyle: sleep, nutrition, exercise.
In case of abnormal blood loss, severe restlessness (mental) or pain: get in touch. If it is an urgent problem, it is best to go to the hospital.
It can take weeks to months before the desired effect is achieved. Together we will look for the best solution for you: this often requires adjustments in dosage, administration form and product.
